CLINIC GUIDELINES


Medication - If you must take medication during school hours, please ask your parent to be sure the school rules are followed:

1. Only medication that cannot be scheduled for other than school hours may be given.

2. All prescription medications must be in the appropriate prescription bottle with a prescription label bearing your name, the name of the medication, and instructions for giving the medication.

3. Non-prescription medicine must be in the original container labeled with your name, the name of the medication, and instructions for giving the medication.

4. All medications must be accompanied by the PISD medical request form completed by your parent.

5. All medications must be stored in the school clinic. Only school personnel may administer medication. You are not to keep medicine with you (in tote-trays, lunch boxes, etc).

6. Any unused medication will be destroyed two weeks after the last dosage. Medication will not be sent home with the student. It may be picked up by a parent at the school clinic.

The rules are necessary for the your safety and the safety of others. Remember, it is YOUR responsibility to come to the office at the necessary time to take your medicine. (Medically challenged children will be monitored when appropriate.) Your parent is required to send a note if you will not be participating in physical education classes due to illness or injury. This note will be accepted up to five consecutive days, longer non-participation will require a physician's written excuse.
