If you are enrolling your child in Haggard Middle School, please
call for an appointment during school hours, 8:00 a.m. – 4:00
p.m. Monday through Friday. You will complete several forms in our
enrollment packet and meet with the grade level counselor.
Appointments
will be made by Registrar, Cissy Lunde at 469-752-5415.
Records necessary for enrollment are:
• Proof of Residence in Plano ISD:
Gas, water or electric utility bill showing the name and address
PLEASE NOTE: Cable
and phone bills cannot be used. OR
Signed Lease contract (followed by a copy of the first utility bill)
OR
Closing paperwork for a new home (followed by a copy of the first
utility bill)
• Student’s original or certified birth certificate
or passport
(we will make copies for you)
• Immunization record signed by a health professional
(click on immunization requirements)
• Copy of student’s social security card or
number
(if available; if not, the student will be assigned a State ID number
until SS# is obtained)
• Parent or guardian driver’s license or passport
• Guardianship paperwork, if applicable
(must be obtained through the PISD Student Services Department –
call 469-752-8021)
• Most recent report card
• Withdrawal papers from current school
(not necessary during summer enrollment)
