General Enrollment Information
As a public school system, the Plano schools welcome all students
who live within the boundaries of the Plano Independent School District
and meet state age and health requirements. Students moving into
the district during the summer may call their school to register
before the start of the school year. Students who move into the
district during the school year should make an appointment to enroll.
A student is required to attend school in the attendance zone in
which the family resides or obtain an approved transfer.
The natural parent, legal guardian or other person having legal
control under a court order must enroll a student. Any person who
is not the natural parent or is not designated guardian of the child
by a court order must register with the office of Student Services
(2700 W. 15th Street, Plano) prior to enrolling the student. The
person enrolling the student must present their driver's license
or department of transportation identification card at the time
of registration. A student 18 or more years of age living independently
must present evidence of age and complete the necessary form in
the office of Student Services prior to enrolling at the school.
If a family has not yet moved into the school district but is building
or purchasing a home within the district boundaries, a Certificate
of Eligibility Form must be completed and the contract for purchase
must be submitted to the office of Student Services prior to enrolling.
Records necessary to enroll are:
Birth Certificate or Passport
Current Immunization Record Signed by a Physician
Proof of Residency in Plano ISD (natural gas or
electricity bill, original lease agreement, or approved Certificate
of Eligibility to Enroll form)
Most Recent Report Card/Withdrawal Slip from Previous
School
Social Security Card (if available; if not, the
student will be assigned a State ID number)
High School Transcript (if applicable)
Parent's Drivers License or Texas Department of
Transportation ID card or Guardian Information Form and Guardian’s
Drivers License or Texas Department of Transportation ID card (if
parent or guardian is not a licensed driver in Texas nor has the
person obtained a TXDOT ID card, other photo ID will be considered
in combination with a birth certificate)
Early Childhood Program Registration
Plano ISD is constructing three Early Childhood Schools to serve
students throughout our school district. Students may enroll based
on three criteria: 1) Tuition-supported; and state and federal programs
including 2) Prekindergarten and 3) Preschool Program for Children
with Disabilities. Click on the following link for program information

Kindergarten Registration
Children must be five years old on or before September 1 to be admitted
to kindergarten. Please bring the following documents in July to
register.
Birth Certificate or Passport
Current Immunization Record Signed by a Physician
Social Security Card
Proof of Residency in Plano ISD (gas or electric
bill)
Parent's or Guardian's Drivers License
First Grade Registration
In order to attend first grade in Plano ISD, a child must be six
years old on or before September 1 or have been previously enrolled
in the first grade of a public school in another state or have completed
public school kindergarten in another state. Children who are underage
but will become six between September 1 and December 31 may enroll
in first grade through the district's kindergarten acceleration
procedures. The child should be registered during the regular Kindergarten
Roundup at his/her school. Parents must attend a district informational
meeting in April and apply for kindergarten acceleration. Assessment
sessions are scheduled in June. Students who move into the district
in July and early August may be considered for acceleration assessment.
The deadline for receiving these students is September 1.

Immunization Requirements 2002-03 (revised 4/30/02)
Texas State Law and Plano School Board Policy state that all required
immunizations be physician or Health Department verified.
| POLIO |
(a) Series of
three (through age 17)
(b) One must have been since the fourth birthday. (May be within
the month prior to or of the 4th birthday) |
| D.P.T./DT: |
(a) Series of four (as of 8-1-96) Seven years and older require
series of three.
(b) One must have been since the fourth birthday. (May be within
the month prior to or of the 4th birthday)
(c) Booster required every 10 years
(d) DTaP will satisfy all DTP immunization requirements at all
ages. |
RUBEOLA:
(Measles) |
(a) Two doses of measles vaccine are required for all students
age 5 years and older who were born on or after September 2,1991.
(b) First dose must be on or after 1st birthday
(c) Proof of immunity or booster dose required by age 12 for
all other students.
(d) ONLY serologic confirmation of measles disease will be acceptable
in lieu of vaccine. |
RUBELLA:
(German Measles) |
(a) One dose of vaccine required on or after first birthday
for all students
(b) ONLY serologic confirmation of immunity will be acceptable
in lieu of vaccine. |
| MUMPS: |
a) One dose vaccine on or after first birthday for all students.
(b) ONLY serologic confirmation of mumps illness will be acceptable
in lieu of vaccine. |
| HIB: |
(a) One dose of vaccine is required for children 15 months
through 4 years unless a schedule for a primary series and a
booster was completed by 15 months of age. |
| HEPATITIS B: |
(a) Three doses are required for all students age 5 yrs. and
older who are born on or after September 1, 1992.
(b) Three doses are required for all students born on or after
September 2, 1988, but before September 1, 1992, by their 12th
birthday or
(c) Serologic confirmation of immunity or serologic evidence
of infection. |
VARICELLA
(Chicken Pox) |
(a) One dose on or after 1st birthday for children born on
or after Sept 1, 1994
(b) Children born on or after Sept 2, 1988, and before Sept
1, 1994, will be required to have one dose at time of their
12th birthday.
(c) Parent or physician validated history of illness or
(d) Serologic confirmation of immunity.
e) If first dose is given after age 13, two (2) doses are required. |
MEDICAL CONTRAINDICATIONS: An affidavit or certificate signed by
a licensed physician stating the required immunization would be
injurious to the health and well being of the student or a member
of his/her household. (Medical contraindications have a one year
limit unless the physician specifies a lifetime condition.)
RELIGIOUS EXEMPTION: A notarized affidavit stating the conflicts.
(This exemption does not apply in times of emergency or declared
epidemic.)
