Forms 1 - 9 listed below must be complete (every signature line) and on file before a student can try out for a sport or join their teammates for practice.
Medical History/Physical Examination Form
- The medical history portion must be filled out every year by a parent/guardian.
- The physical examination portion must be completed by a doctor if not on file from 2008-2009.
