General Enrollment Information
As a public school system, the Plano schools welcome all students
who live within the boundaries of the Plano Independent School
District and meet state age and health requirements. Students
moving into the district during the summer may call their school
to register before the start of the school year. Students who
move into the district during the school year should make an appointment
to enroll. A student is required to attend school in the attendance
zone in which the family resides or obtain an approved transfer.
The natural parent, legal guardian or other person having legal
control under a court order must enroll a student. Any person
who is not the natural parent or is not designated guardian of
the child by a court order must register with the office of Student
Services (2700 W. 15th Street, Plano) prior to enrolling the student.
The person enrolling the student must present their driver's license
or department of transportation identification card at the time
of registration. A student 18 or more years of age living independently
must present evidence of age and complete the necessary form in
the office of Student Services prior to enrolling at the school.
If a family has not yet moved into the school district but is
building or purchasing a home within the district boundaries,
a Certificate of Eligibility Form must be completed and the contract
for purchase must be submitted to the office of Student Services
prior to enrolling.
Records necessary to enroll are:
- Birth Certificate or Passport
- Current Immunization Record Signed by a Physician
- Proof of Residency in Plano ISD (natural gas or electricity bill,
original lease agreement, or approved Certificate of Eligibility
to Enroll form)
- Most Recent Report Card/Withdrawal Slip from Previous School
- Social Security Card (if available; if not, the student will be
assigned a State ID number)
- Parent's Drivers License or Texas Department of Transportation
ID card or Guardian Information Form and Guardian’s Drivers
License or Texas Department of Transportation ID card (if parent
or guardian is not a licensed driver in Texas nor has the person
obtained a TXDOT ID card, other photo ID will be considered in
combination with a birth certificate)
Early Childhood Program Registration
Plano ISD is constructing three Early Childhood Schools to serve
students throughout our school district. Students may enroll based
on three criteria: 1) Tuition-supported; and state and federal
programs including 2) Prekindergarten and 3) Preschool Program
for Children with Disabilities. Click on the following link for

Kindergarten Registration
Children must be five years old on or before September 1 to be
admitted to kindergarten.
Please bring the following documents
in July to register.
- Birth Certificate or Passport
- Current Immunization Record Signed by a Physician
- Social Security Card
- Proof of Residency in Plano ISD (gas or electric bill)
- Parent's or Guardian's Drivers License

First Grade Registration
In order to attend first grade in Plano ISD, a child must be six
years old on or before September 1 or have been previously enrolled
in the first grade of a public school in another state or have
completed public school kindergarten in another state. Children
who are underage but will become six between September 1 and December
31 may enroll in first grade through the district's kindergarten
acceleration procedures. The child should be registered during
the regular Kindergarten Roundup at his/her school. Parents must
attend a district informational meeting in April and apply for
kindergarten acceleration. Assessment sessions are scheduled in
June. Students who move into the district in July and early August
may be considered for acceleration assessment. The deadline for
receiving these students is September 1.

Immunization Requirements
2006-2007

Texas law requires proof of immunization against Diphtheria, Tetanus, Pertussis, Poliomyelitis, Hepatitis B, and HIB, and proof of immunization or a history of illness from Rubella, Rubeola, Mumps and Varicella. Pre-K students are required to have Hepatitis A and Pneumococcal. This is required of every student, according to prescribed school grade criteria, seeking admission to or enrolled in any school in the Plano Independent School District.

POLIO
Four doses are required, one of which must be on or after the fourth birthday. If the 3rd dose was given on or after the fourth birthday, only 3 doses are required. If any combination of four doses of OPV and IPV was received before four years of age, no additional dose is required. Receipt of the dose up to and including 4 days before the 4th birthday will satisfy the school entry immunization requirement. If 18 years or older, not required.
D.P.T./DT
Five doses of any combination DTaP/DTP unless 4th dose was received on or after the 4th birthday in which case only 4 doses are required. Students who started their vaccinations after age 7 are required to have at least 3 doses of a tetanus-diphtheria containing vaccine. Receipt of the dose up to and including 4 days before the 4th birthday will satisfy the school entry immunization requirement. One dose of Td required ten years after last dose of DTP/DTaP/DT.
MMR(Measles, German Measles, Mumps, Rubella, Rubeola)
Two doses of a measles-containing vaccine with the 1st dose on or after the 1st birthday; 2nd dose by age 5 or entry into kindergarten. One dose of rubella vaccine and one dose of mumps vaccine received on or after the 1st birthday are required. Receipt of the dose up to and including 4 days before the 1st birthday will satisfy the school entry immunization requirement.
Serologic confirmation of immunity to measles, mumps, rubella, or serologic evidence of
infection is acceptable in lieu of vaccine.
RUBELLA
(German Measles)
(a) One dose of vaccine required on or after first birthday for all students
(b) ONLY serologic confirmation of immunity will be acceptable in lieu of vaccine.
MUMPS
(a) One dose vaccine on or after first birthday for all students.
(b) ONLY serologic confirmation of mumps illness will be acceptable in lieu of vaccine.
HIB
Required for children age 15 months through 4 years. One dose on or after age 15 months, unless scheduled for a primary series (2 doses before the 1st birthday and a booster after the 1st birthday).
HEPATITIS B
Three doses are required for the following grades in the following school years:
2004-05, K-5 and 7-10
2005-06, K-11
2006-07, K-12
Serologic confirmation of immunity to hepatitis B or serologic evidence of infection is
acceptable in lieu of vaccine.
Two doses of adult hepatitis B vaccine (Recombivax) are acceptable. Dosage and type of
vaccine must be clearly documented (Two 10mcg/1.0ml of Recombivax).
Pre-k students, two doses given at least 6 months apart and no longer than 18 months from the date of the first dose.
HEPATITIS A
Two doses given at least 6 months apart and no longer than 18 months from the date of the first. Is not yet required but is highly recommended.
VARICELLA (Chicken Pox)
One dose on or after the 1st birthday for the following grades in the following school years:
2004-05, K-4 and 7-10
2005-06, K-5 and 7-11
2006-07, K-12
Two doses if vaccine given at 13 years of age or older.
Serologic confirmation of immunity to varicella or documentation of previous illness may
substitute for vaccination.
For Pre-k students, a series of three shots given prior to age 1 with a booster after 1 year. If no prior doses given, one dose required. Not given after age 5.

MEDICAL EXEMPTION: An affidavit or certificate signed by a licensed physician stating the required immunization would be medically harmful or injurious to the health and well being of the child. The written statement by the physician must be submitted to the school. Unless it is written in the statement that a lifelong condition exists, the signed statement of exemption is good for one year only from the date signed by the physician.
CONSCIENTIOUS/RELIGIOUS EXEMPTION: The law allows parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief. Instructions for the notarized affidavit can be found at www. immunizeTexas.com. The completed and returned form from the TDH must be submitted to the school. The affidavit will be valid for two years.
PLEASE NOTE: In the case of an outbreak, an un-immunized child may be prohibited from attending school.
