Registration Information

If you plan to enroll your child at Jackson Elementary School School, you will need the following information to complete the registration process.

1. Certified birth certificate or passport.

2. Shot Records from the doctor's office with the doctor's signature/clinic seal visible.*

3. An original Social Security Card. If not available, please order a duplicate through your local Social Security Office. Registration does not hinge on this requirement.

4. Proof of Residency (electric or gas bill, copy of lease, renter's agreement or mortgage papers).

5. Parents Driver's License

Please come by the school and complete the registration packet between the hours of 7:30 - 11:45 a.m. and 1:00 - 3:15 p.m. This will take only 10-15 minutes. If you have any questions, please call the front office at 469-752-2100 and we will be happy to discuss concerns with you. Tours of the school are available by appointment.
