Immunization Requirements

Texas State Law and Plano School Board Policy states that all required
immunizations be physician or Health Department validated.


D.P.T./DT (a) Series of five (as of 8-7-03). If fourth dose was given on or after the fourth birthday, no fifth dose required. (Dose may be given within the calendar month prior to or of the fourth birthday.) (Any combination of diphtheria, pertussis, tetanus vaccine is acceptable.)
(b) Students who begin their vaccinations after age seven require a series of three doses. (Any combination tetanus/diphtheria vaccine is acceptable.)
(c) Booster required every 10 years.
POLIO (a) Series of four, one dose must have been given on or after the fourth birthday (as of 3-11-04).
(b) If third dose was given on or after the fourth birthday, no fourth dose required. (Dose may be given within the calendar month prior to or of the fourth birthday.)
(c) If any combination of four doses of OPV & IPV is given before the fourth birthday, no additional dose is required.
(d) Polio vaccine is not required for students 18 years or older.
RUBEOLA
(Measles)
(a) ALL students, two doses of vaccine required on or after the first birthday. (As of October, 2002, doses given up to four days prior to the first birthday will be accepted.)
(b) ONLY serologic confirmation of measles disease will be accepted in lieu of vaccine.
RUBELLA
(German Measles)
(a) ALL students, one dose of vaccine required on or after first birthday (As of October, 2002, doses given up to four days prior to the first birthday will be accepted.)
(b) ONLY serologic confirmation of German measles disease will be accepted in lieu of vaccine.
MUMPS (a) ALL students, one dose of vaccine required on or after first birthday (As of October, 2002, doses given up to four days prior to the first birthday will be accepted.)
(b) ONLY serologic confirmation of mumps illness will be accepted in lieu of vaccine.
VARICELLA
(Chicken Pox)
(a) One dose on or after the first birthday for all students in grades Kindergarten – 4th and grades 7th – 10th. (As of October 2002, doses given up to four days prior to the first birthday will be accepted.)
(b) Parent or physician validated history of illness; or
(c) Serologic confirmation of immunity.
(d) If first dose of vaccine is given after age 13 years, two doses are required.
HIB (a) Required for children 15 months through 4 years. One dose on or after 15 months unless a schedule for a primary series (2 doses) and a booster (after the first birthday) was given.
HEPATITIS B (a) Three doses are required for all students in grades Kindergarten – 5th and grades 7th – 10th
(b) ONLY serologic confirmation of immunity or serologic evidence of Hepatitis B infection will be accepted in lieu of vaccine.
 
MEDICAL CONTRAINDICATIONS: An affidavit or certificate signed by a licensed physician stating the required immunization would be injurious to the health and well being of the student or to a member of his/her household. (Medical contraindications are valid for one year unless the physician specifies a lifetime condition.)
 
CONSCIENTIOUS/RELIGIOUS EXEMPTION: Parent must apply in writing to: Texas Department of Health, Immunization Division, 1100 W. 49th St., Austin, TX 78756. (Conscientious/Religious Exemption must be renewed every two years (as of 3-11-04) and is invalid in times of emergency or declared epidemic.)
