KINDERGARTEN REGISTRATION  2009-2010

Eligibility: Students must be 5 years old as of September 1, 2009

Required documents:

1. Birth Certificate (must be original with the state seal).

2. Immunization Record (must meet Plano ISD requirements).

    (Please see the reverse side of this page for immunization requirements).

3. Proof of Residency (current gas or electric bill with your name and                    

    address on the bill).

  • Lease or closing statement for home/apartment if first gas or electric bill has not been received.
  • If you have a contract/lease agreement on a property, but your closing date is later in the school year, you must apply for a Certificate to Enroll in Plano Schools. For this application please go to the administration building at 2700 W. 15th Street, Plano 75075.  Please ask for Student Administrative Services.  You will need to take your contract/lease agreement with you.

4. Driver’s License.

5. Social Security card if available. (Not necessary to complete registration).

***No registration will be processed without documentation numbers 1 - 4***

Note: Stinson PTA will take orders for school supplies.  Pre-payment is required. This is an optional service provided to our families.

Tours of the building will not be available on registration day.  Please call the school and make arrangements to visit us at another time.                                                                                                                               

To register your child at Stinson Elementary, please provide the following:

1. Certified copy of child’s birth certificate
2.  Immunization record signed by a physician
3.  Proof of residency (one of the following)
  a. Current gas or electric bill showing your printed name and address
  b. Original closing papers or lease agreement
  c. If you have not yet moved into your home a Certificate of Eligibility is needed to enroll in PISD from the district’s central office. Call 469-752-8232 for directions on that process.
  d. If you are living with someone who resides within the attendance area, a notarized proof of residency form is required along with one of the items mentioned above.
4. Social Security Card (if available).
5. Parent's driver's license.

The parent or legal guardian is responsible for providing all information before enrollment can be completed. If not a parent, legal proof of guardianship is required.

Age Requirements

 Kindergarten:

Children must be five years old on or before September 1 to be admitted to kindergarten.

First Grade:

To attend first grade in Plano ISD, a child must be six years old on or before September 1 or have been previously enrolled in the first grade of a public school in another state or have completed public school kindergarten in another state. Children who are underage but will become six between September 1 and December 31 may enroll in first grade through the district's kindergarten acceleration procedures. The child should be registered during the regular Kindergarten registration at his/her school.

Kindergarten Acceleration - In order to apply for the testing process, parents must attend a parent information meeting through the Plano ISD. You may check with the front office of the school your child will attend to get more information on this meeting.

Health Information

Immunization Requirements for 2009-2010:

Texas law requires proof of immunization against Diphtheria, Tetanus, Pertussis, Poliomyelitis, Hepatitis B, and HIB, and proof of immunization or a history of illness from Rubella, Rubeola, Mumps and Varicella. Pre-k students are required to have Hepatitis A and Pneumococcal. This is required of every student, according to prescribed school grade criteria, seeking admission to or enrolled in any school in the Plano Independent School District.

Diphtheria/Pertussis/Tetanus/Td
(DTP, DTaP, DT, Td)
  • Five doses of any combination DTaP/DTP unless 4th dose was received on or after the 4th birthday in which case only 4 doses are required. Students who started their vaccinations after age 7 are required to have at least 3 doses of a tetanus-diphtheria containing vaccinen provided one dose was on or after the fourth birthday. Tdap required for 7th grade beginning 2009-2010 unless proof of booster within last five years of tetanus containning vaccine. Grade 8-12 beginning 2009-2010 who have not already received Tdap are required to receive one booster dose of Tdap when ten years have passed since the last dose of a tetanus-diphtheria containing vaccine.
Polio
  • Four doses are required, one of which must be on or after the fourth birthday. If the 3rd dose was given on or after the fourth birthday, only 3 doses are required. Receipt of the dose up to and including 4 days before the 4th birthday will satisfy the school entry immunization requirement. If any combination of four doses of OPV and IPV was received before four years of age, no additional dose is required.
    If 18 years or older, not required.
Measles, Mumps, Rubella (MMR)
  • Beginning 2009-2010 two doses of MMR for Kindergarten entry with the fist dose on or after the first birthday.
  • Serologic confirmation of immunity to measles, mumps, rubella or serologic evidence of infecction is acceptable in lieu of vaccine.

Hepatitis B

 

 

 

 

Hepatitis A

  • Three doses are required for grades K-12.
  • Serologic confirmation of immunity to hepatitis B or serologic evidence of infection is acceptable in lieu of vaccine.
  • Two doses of adult hepatitis B vaccine (Recombivax) are acceptable. Dosage and type of vaccine must be clearly documented (Two 10mcg/1.0ml of Recombivax).

 

  • One dose administered after one year of age. Second dose administered at least 6-12 months apart.
  • Beginning 2009-2010 two dose requirement for Kindergarten entry with the first dose received on or after the first birthday.

Varicella (Chickenpox)

 

 

 

 

Meningococcal

 

Pneumococcal

  • One dose is required for grades K-12. Receipt of the dose up to and including four days before the 4th birthday.
  • Beginning 2009-2010 two dose requirement for Kindergarten and 7th grade with the first one on or aft the first birthday.
  • Two doses if vaccine given at 13 years of age or older.
  • Serologic confirmation of immunity to varicella or documentation of previous illness may substitute for vaccination.

 

  • Beginning 2009-2010 one dose required for 7th grade.

 

  • For Pre-K students, one dose on or afer second birthday if series not completed. If over age one or less than two will need series. Age two to five must have one shot if not completed. Not given after age five.
HIB
  • Required for children age 15 months through 4 years. One dose on or after age 15 months, unless scheduled for a primary series (2 doses before the 1st birthday and a booster after the 1st birthday).

Medical Exemption: The law allows physicians to write a statement stating that the vaccine(s) required would be medically harmful or injurious to the health and well being of the child. The written statement by the physician must be submitted to the school. Unless it is written in the statement that a lifelong condition exists, the exemption statement is valid for only one year from the date signed by the physician.

Conscientious/Religious Exemption: The law allows parents/guardians to choose an exemption from immunization requirements for reasons of conscience, including a religious belief. Instructions for the affidavit to be signed by parents/guardians can be found at www.ImmunizeTexas.com. The completed and returned form from the TDH must be submitted to the school. The affidavit will be
valid for two years.

Please note: In the case of an outbreak, an un-immunized child may be prohibited from school attendance.
